NEW MEMBER DOCUMENTS

MECHANICSBURG FIRE DEPARTMENT

 

 

The below linked items must be completed and returned with your Membership Application before being considered for membership:

• Membership Application

• Beneficiary Designation Form

• Personal Information Form

• Fingerprint Waiver Request

  • ​Can only be accepted if you have lived in Pennsylvania for the entire previous 10 years. If not, you will have to schedule a fingerprinting.

• I Am Responding Registration Form

We will also need the results from the following 2 websites prior to being able to process your Membership Application:

• PA State Police Background Check

  • Ensure you select the "New Record Check - Volunteers only" option 

• A Child Abuse Clearance

We will also need the following, if applicable:

• Copies of any appropriate certificates

• A copy of your current PA Drivers License

 

For any new members under the age of 18, a copy of your working permit, signed by a representative of the Borough of Mechanicsburg
